How to Get Records

Mail

For mail requests, you must first complete a form 431069. The cost is $5.50, and there is no service charge. Traffic ticket fees are fifty cents per copy. When ordering your own records, just fill out sections "A" and "D." Checks should be written to "Treasurer, State of Iowa" and mailed to:
"Office of Driver Services Iowa Department of Transportation P.O. Box 9204 Des Moines, Iowa 50306-9204"
